General Information

AHS will not accept form submissions without attached attendance notes.
Excuse notes (other than College/Postsecondary Visits and absences for Non-School Sponsored Extracurricular Activities) MUST be submitted within 5 days upon returning to school after an absence or the absence will remain unexcused (per ACS Board Policy 6.200)
Students should check PowerSchool regularly to ensure all properly submitted attendance notes are processed.

College/Postsecondary Visits

College/post-secondary visits are not required of any student. Parents/guardians are solely responsible for facilitating visits and the safety of their student(s).
"College/post-secondary" can mean a 2-year or 4-year institution, a vocational/technical school, or Military Entrance Processing Station (MEPS).
Requests for college/post-secondary visit days require a written request be submitted no later than seven (7) days prior to the student's absence, per ACS Board Policy 6.200.
Request notes may be uploaded using the form below and must include: the student's name, grade, date(s) of visit and any travel days, institution you will be visiting, and both student and parent signatures.
The student must submit written verification from a campus official of the post-secondary institution no later than two (2) days of the post-secondary visit.
Students approved for a college/post-secondary visit will be marked FT for the day of the visit, with a maximum of four (4) school days per year. For each visit, up to two (2) days will be excused for travel. Travel days will be marked as excused absences.
Students are responsible for completing any work missed during a college/post-secondary visit within five (5) school days of the student's return to school.

Requests for excused absences for non-AHS extracurricular activities (e.g., club sports, club cheer/dance, etc.) require a written request be submitted no later than seven (7) days prior to the student's absence, per ACS Board Policy 6.200.
Request notes may be uploaded using the form below and must include: the student's name, grade, date(s) of absence, reason for absence, proof of participation in the non-school sponsored activity and both student and parent signatures.
All absences must also be verified in writing by the parent within two (2) days of the student's return to school.
